Parent Permission

I hereby consent to my child participating in all KidSingers activities at the First Presbyterian Church in Santa Ana and performances and field trips off the premises that are a part of KidSingers activities. I also understand that KidSingers is a 9-month commitment from September 2023 to May 2024. We expect all accepted KidSingers to abide by this commitment and to make KidSingers their top priorities for Tuesday afternoons. Should it be necessary for my child(ren) to have medical treatment during KidSingers participation time, I hereby give KidSingers directors and staff permission to use their judgment in obtaining medical service for my child(ren). I give permission to the physician selected by KidSingers personnel to give medical treatment deemed necessary and appropriate by the physician. I agree that in the event my child is injured as a result of his/her participation at KidSingers, including transportation to and from any activity, the administrative staff, directors, volunteers, chaperones, and KidSingers will be held harmless. I hereby authorize KidSingers to photograph, or permit other persons to photograph, my child(ren), and agree that they may use the negatives, prints, videotapes, or DVDs as it pertains to the public relations and fundraising efforts of KidSingers (for example in concerts, programs, newspapers, social media, etc.). KidSingers is responsible for supervising your child(ren) as long as they are on First Presbyterian Church premises for rehearsals. If your child leaves the rehearsal premises, KidSingers will not be held responsible for your child(ren). I understand that KidSingers will not be held responsible for my child(ren) if he/she leaves the rehearsal premises with or without my permission.
